Luke's Thank-You Message to the Division of Specialized Care for Children

We want to share a sweet thank-you message we received from one of our participant families in the Home Care Program.

The Home Care Program serves children and adults who require in-home shift nursing. Many of these children and young adults are medically fragile and depend on technology for their well-being.

The program helps participants live safely in their family home rather than in a hospital or skilled nursing facility. We operate the Home Care program on behalf of the Illinois Department of Healthcare and Family Services (HFS).

When a family applies for Home Care services for their child, we assign the family a Home Care Enrollment Specialist. The Division of Specialized Care for Children (DSCC) Enrollment Specialist guides the family through the application process.

The DSCC Enrollment Specialist also helps the family prepare for their child’s transfer home and provides support along the way.

Yoran St. Clair is the Enrollment Specialist for Luke Joshua Hougan, a 9-month-old infant born with complex medical needs.

Yoran helped Luke’s parents successfully apply for the Home Care Program. She’s also worked closely with the family to help them plan for Luke’s discharge from the hospital so they can safely care for him at home.

Luke’s mom, Christina, appreciates Yoran’s hard work and help. She recorded a special video of thanks that you can watch below.

“Thank you for all of the hard work you’ve done to help us get Luke home and for doing all of his paperwork and for helping his mama,” Christina says in the video on Luke’s behalf.

This message is a wonderful testament to Yoran’s care coordination for this family and the valuable role of our Home Care Enrollment Team!
